Job Summary
Upperline Health is seeking a Nurse Practitioner to provide clinic-based care, develop complex plans of care, and offer medical diagnostics and treatments. Responsibilities include gathering patient histories, implementing treatment plans, and educating patients and families on various healthcare topics. Candidates should have a Master's degree in nursing, board certification, and two years of primary care experience. The role requires strong communication skills, adaptability, and the ability to work in a fast-paced environment.
Required Qualifications
- Master’s degree in nursing required
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required
- Current Florida State Registered Nurse & Nurse Practitioner license required
- A minimum of two (2) years Nurse Practitioner experience in primary care / internal medicine
- Basic Life Support (BLS) certification is required
- Possession of DEA registration or eligibility
Desired Qualifications
- Experience with primary care / internal medicine
- Experience with geriatric care and/or home-based care
- Experience with palliative or hospice care
- Value-based care experience
- Previous experience managing, supervising, or coaching clinicians
- Experience working in an EMR; AthenaHealth EMR preferred
- Proficiency using Microsoft Office products
Additional Requirements
- Must be comfortable providing care in patients' homes
- A reliable automobile, valid driver’s license and minimum state required liability auto insurance are required
