Job Summary
Develop and implement clinical plans of care for adult patients with chronic conditions like diabetes, COPD, and CHF, gathering history and performing physical exams in clinic or home settings. Provide medical diagnostics and treatments, prescribe medication as permitted, and educate patients and families on medication usage, diet, nutrition, and crisis prevention. Collaborate with the interdisciplinary team and local physicians to coordinate optimal care, review patient records timely, and communicate medical information across providers. Maintain accurate medical documentation per risk adjustment standards, participate in weekly team meetings, and provide telephonic support for escalated triage calls. Complete continuing education and BLS certification as required.
Required Qualifications
- Master's degree in nursing
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner
- Current State Registered Nurse & Nurse Practitioner license
- A minimum of one (1) years Nurse Practitioner experience in primary care / internal medicine
- Value-based care experience
- Possession of DEA registration or eligibility
- Basic Life Support (BLS) certification
- Must be comfortable providing care in patients homes
- A reliable automobile, valid driver's license and minimum state required liability auto insurance
- Adaptability to thrive is a fast-paced growth-forward environment
- Experience working in an EMR
- Proficiency using Microsoft Office products
- Excellent oral and written communication skills
- Must be able to lift 50 lbs
Desired Qualifications
- Previously worked in a value-based care environment
- Experience with the following: primary care / internal medicine, geriatric care and/or home-based care, palliative or hospice care
- Previous experience managing, supervising, or coaching clinicians
- Experience working in an EMR; AthenaHealth EMR is highly preferred
