Nurse Practitioner

Nurse Practitioner sought to provide clinic-based care in the Villages, FL, collaborating with Upperline Health’s physician team to develop and implement comprehensive care plans for adults with chronic conditions in clinic and home settings. Responsibilities include gathering patient history, exams, and diagnostics in home/clinic, creating treatment plans aligned with patient goals, coordinating with an interdisciplinary team to address physical and psychosocial needs, educating patients and families on medications, side effects, illness progression, diet, and crisis prevention, communicating with physicians and the broader care team, maintaining medical records per regulations, timely documentation per risk-adjustment standards, participating in weekly team meetings and education sessions, providing telephonic support for escalated calls, continuing education per state requirements, prescribing medications as permitted by the state Board of Nursing, and performing related duties. Required: Master’s in nursing, NP board certification (Adult, Adult-Geriatric, or Family), active RN/NP licenses, minimum 6 months NP experience in primary care/internal medicine, BLS, DEA eligibility or registration, ability to provide in-home care, and experience with EMR (AthenaHealth preferred).
