Job Summary
Provide primary, urgent, and preventive medical care to residents in skilled nursing and assisted living settings. Independently evaluate, diagnose, and treat acute and chronic conditions while conducting admissions, follow-up assessments, and clinically appropriate screenings. Monitor resident changes, adjust treatment plans, and order/interpret diagnostic tests to prescribe medications within scope. Collaborate with Medical Directors, attending physicians, and interdisciplinary teams to coordinate care and communicate diagnoses to families. Maintain accurate electronic medical record documentation and adhere to regulatory standards. Participate in quality improvement efforts to support excellent patient outcomes.
Required Qualifications
- Active, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
- Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
- Clinical training and experience in adult, family, and/or geriatric medicine.
- Eligibility to work in the United States.
Desired Qualifications
- Experience in internal medicine, geriatrics, cardiology, or post-acute care.
- Active DEA registration or ability to obtain.
- Strong clinical judgment and ability to work independently.
- Excellent communication and interpersonal skills.
- Organized, detail-oriented, and able to manage multiple priorities effectively.
- Prior experience in SNF, LTC, ICU, or Emergency Department settings is a plus.
- Ability to collaborate effectively within interdisciplinary care teams to optimize resident outcomes.
