Job Summary
Provide outpatient primary care to a Medicare-aged, medically underserved population with comorbid conditions, managing medical and social needs through examination methodologies and diagnostics. Collaborate with Family Practice and Internal Medicine physicians, RN Clinical Managers, and center team members within state-of-the-art, newly built community centers. Deliver comprehensive care focused on continuity and patient comfort rather than service volume, utilizing cloud-based electronic health records. Travel within the market to multiple centers located in short distance of each other.
Required Qualifications
- Currently licensed to practice professional nursing or Physician Assistant in the state of employment, active and non-probationary
- Experienced in primary care: internal medicine, geriatrics, or family medicine
- Current BLS/ACLS certification
- Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
- Strong understanding of examination methodologies and diagnostics
- Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members
- Confident in their ability to deliver high quality primary care, which includes being available to patients and addressing their medical and social needs
- Comfortable traveling within the market to multiple centers within short distance of each other
