Job Summary
Remote, full-time Nurse Practitioner focused on Transitional Care Management (TCM). Conduct post-discharge visits and patient assessments, review discharge summaries and treatment plans, perform medication reconciliation, develop individualized care plans, monitor patient progress, coordinate with physicians and home health/hospice providers, educate patients and caregivers, and document encounters in eClinicalWorks. Support quality, compliance, and patient outcome initiatives while working remotely with a multidisciplinary team.
Required Qualifications
- Active and unrestricted Texas Nurse Practitioner license
- National Board Certification
- DEA license or ability to obtain
- Strong clinical assessment, communication, and documentation skills
- Ability to work independently in a remote environment
