Job Summary
Establish new markets by traveling to regions to launch local care teams, setting up operational workflows, and training clinicians on company protocols. Deliver direct patient-centered care in home settings, including health assessments, diagnostic testing, therapeutic interventions, and medication management. Develop personalized care plans, monitor patient progress, and provide education to patients and families while assessing market needs to address care gaps. Collaborate with local leadership and community resources to standardize operations and ensure high-quality delivery. Requires a current Nurse Practitioner license, DEA, and ability to drive up to 150 miles daily in varying weather conditions.
Required Qualifications
- Current Nurse Practitioner License
- Must be able to obtain and maintain a Nurse Practitioner License within all service states within 6 months of hire
- Must have an active DEA license or be eligible to obtain DEA license
- Active CPR or BLS Certification
- Must maintain a valid driver's license and good driving record
- Outstanding EHR skills
- Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings
- Ability to drive, if necessary
- High flexibility and ability to travel for extended periods of time
- Full range of body motion including handling/lifting patients
- Manual and finger dexterity
- Eye-hand coordination
- Normal visual acuity
