Job Summary
Nurse Practitioner – Telehealth role delivering primary care and care management to high-risk patients via virtual visits from home, with in-home and facility-based coordination across South Carolina and Georgia. Responsibilities include evaluating high-risk patients, developing individualized care plans, coordinating care with interdisciplinary teams, communicating with case managers and providers, documenting activities for regulatory compliance, educating patients and families, and supporting HIPAA/Medicare guidelines. Full-time position with bonus opportunities, flexible hours, and travel between offices as needed. Must hold an active NP/PA/Physician license in the employment state and be able to practice with good standing; strong collaboration, organizational, and patient-centered care capabilities are required.
Required Qualifications
- Nurse Practitioner (NP), Physician Assistant (PA), or Physician with a valid license to practice in the state of employment required
- License must be in good standing
- Experience in home health, clinic, facility, telehealth, or specialty care setting preferred
- Strong communication and interpersonal skills; ability to work within an interdisciplinary team
- Ability to evaluate high-risk patients, develop care plans, and coordinate care activities
- Willingness to travel between offices/facilities as needed
- Licensed driver with automobile insured and in good working order
