Job Summary
Nurse Practitioner, Team Lead providing non-urgent primary care assessments in person or via telemedicine, leading a clinical team of nurses, delivering patient education and coaching, and collaborating with interprofessional teams to plan and follow up care for clients with substance use, mental health, and gambling concerns. Responsibilities include direct clinical assessments within the RN extended class scope, telemedicine assessments for urgent needs, staff training and development, program oversight, data collection and reporting, service improvements, and maintaining harm-reduction, trauma-informed, women-centred, and anti-oppressive practices across organizational operations.
Required Qualifications
- Master’s Degree in Nursing
- Current registration and in good standing with the College of Nurses of Ontario
- Nurse Practitioner Certification from a recognized program
- Membership with the Nurse Practitioner’s Association of Ontario (NPAO)
- Minimum of 2-4 years of clinical experience in a hospital or community health setting, including 2 years caring for clients and families with substance use, mental health and gambling concerns
