Job Summary
Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans. Prescribe medications and treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations. Collaborate with interdisciplinary teams and physicians to manage symptoms while educating patients, families, and caregivers on end-of-life needs. Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives. Requires current NP or APRN license, accredited degree, and two years of RN experience with preferred hospice background. Valid driver's license and reliable transportation required. Schedule is 8:00am to 5:00pm M-F.
Required Qualifications
- Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice.
- Degree from an accredited NP program
- Minimum two years of RN experience
- Valid driver's license
- Reliable transportation
- Automobile insurance
- Ability to pass background and drug screening

- Preferred hospice or palliative care experience
