Job Summary
Provide full-scope primary health care to clients in supportive housing and medical stabilization settings as part of an interprofessional team. Responsibilities include comprehensive triage, physical assessment, diagnosis and treatment, counselling, health promotion, disease prevention, and referral to specialists or diagnostic testing. Deliver care aligned with standards of practice for extended-class nurses and expanded nursing services, while balancing risk, harm reduction principles, and client-centered approaches. May require after-hours and weekend coverage; opportunities for outreach with community partners and ongoing evaluation of medical directives to support safe, responsive care across community and housing settings.
Required Qualifications
- MScN RN(EC) or equivalent
- Current Extended Class certificate (NP-PHC) and unrestricted registration with the College of Nurses of Ontario
- Minimum two years’ experience as a nurse practitioner in a community or primary care setting
- Strong clinical skills and diagnostic abilities
- Experience with culturally diverse and marginalized populations
- Ability to work independently and as part of an interprofessional team
- Knowledge of harm reduction approach and service delivery
- Strong interpersonal and communication skills
- Valid CPR and First Aid
- Police record check for vulnerable populations
- Language capability: English, French; proficiency in other languages considered an asset
