Job Summary

Provide comprehensive primary care services to urban Indigenous clients, including physical exams, chronic disease management, antenatal care, and acute visits. Conduct on-site assessments, telephone consultations, and outreach visits while reviewing lab results and arranging referrals. Advocate for equitable access to traditional and western healing services by collaborating with physicians, allied health professionals, and community partners. Document all client interactions confidentially and maintain close working relationships within the interdisciplinary team. This role supports SOAHAC's mandate to build health care capacity within Southwest Ontario Indigenous communities, offering 35 hours weekly with a salary of $117,960 to $147,350.

Required Qualifications

  • BScN
  • current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
  • Primary Health Care Nurse Practitioner Certificate
  • 2-3 years' previous experience working as a Nurse Practitioner in the areas of primary care, chronic disease management, mental health and addictions
  • Knowledge of culture and local Indigenous communities in Southwestern Ontario
  • Strong organizational skills with the ability to work effectively and independently
  • Excellent communication skills
  • Strong attention to detail and assessment skills
  • Ability to work effectively as part of an interdisciplinary team
  • Ability to establish and maintain effective working relationships with others and across teams
  • Excellent interpersonal skills with the ability to communicate clearly
  • Computer literacy including skills in word processing, e-mail, PowerPoint, Excel, and Electronic Medical Records
  • Valid Ontario Driver's "G" license
  • personal auto insurance (at least 3 years experience)
  • clean driver's abstract
  • Obtain/ maintain a current and satisfactory police check
  • Up-to-date immunizations and records
