Job Summary
Nurse Practitioner for the ISNP program delivering coordinated, comprehensive care at designated skilled nursing facilities. Collaborates with the member’s primary care physician to create care plans focused on prevention, early detection, and timely intervention; serves as the medical liaison to the interdisciplinary care team; ensures adherence to PPHP documentation and care standards; performs autonomous health assessments, orders/ interprets diagnostics, prescribes medications, and supports transitions of care with a focus on elderly members and families.
Required Qualifications
- Current license as a Nurse Practitioner (Acute, Adult/Gerontological or Family) in the practicing state
- Meets Provider Partner Health Plan credentialing requirements
- DEA licensure and CDS licensure (if required by state)
- Basic Cardiac Life Support (BCLS) certification
- Current unrestricted RN license for the state of practice
- Valid driver’s license and car insurance with reliable transportation
- Minimum of 3 years clinical care experience
- Experience in SNF/LTC preferred
- Case management experience preferred
- Knowledge of Medicare/Medicaid processes
- Knowledge of I-SNP MOC and population
- Proficiency with nursing/medical databases and Microsoft programs
