Job Summary

Provide autonomous Nurse Practitioner clinical care in skilled nursing facilities as part of the Institutional Special Needs Plan (ISNP) and Provider Partner Health Plan (PPHP) program. Collaborate with the member’s primary care physician to deliver preventive, acute, and chronic care; conduct comprehensive health assessments; order and interpret diagnostics; prescribe medications; refer to specialists as appropriate; establish a care plan with short-term and long-term goals; serve as a medical liaison to the interdisciplinary staff and families; ensure a seamless plan of care, transitions of care, and avoidance of unnecessary testing. Maintain documentation per PPHP standards, observe scope of practice in the licensed state, and participate in continuous quality improvement. Required proficiencies include clinical expertise, evidence-based practice, strong computer and data-reporting skills, exceptional interpersonal and teaching abilities, and a commitment to elderly patient care. Minimum 3 years clinical care experience; SNF/Long Term Care experience and case management experience preferred; knowledge of Medicare/Medicaid and I-SNP requirements; current DEA and CDS credentials if required; valid RN license in practice state; DEA/CDS and BLS as applicable; valid driver’s license and reliable transportation. This role emphasizes autonomy, collaboration with physicians and families, and effective care coordination within the PPHP/ISNP framework.

Required Qualifications

  • Minimum of 3 years clinical care experience
  • Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice
  • Meets the Provider Partner Health Plan credentialing requirements
  • Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
  • Basic Cardiac Life Support certification
  • Current unrestricted RN license for the state in which the Nurse Practitioner is practicing
  • Valid driver’s license and car insurance with reliable transportation
  • Knowledge of Medicare/Medicaid and I-SNP MOC and I-SNP population
  • Proficiency in nursing/medical databases and Microsoft programs
  • Ability to operate autonomously within scope of practice
  • Experience with transitions of care and working with interdisciplinary teams
  • Strong communication and teaching skills
  • Attention to documentation standards as directed by PPHP Model of Care
  • Healthcare experience in long-term care or skilled nursing facilities preferred
