NURSE PRACTITIONER - Small Panel- High Acuity

Collaborate with 3-5 established PCPs to co-manage a small panel of high-risk adult patients in the SHARP model, providing enhanced, high-touch, independent, and collaborative primary care. Deliver extended visits with significantly lower visit volumes than standard practices while working beside Johns Hopkins nurses and staff. Manage clinical duties alongside 8 hours of eligible remote admin time, supported by dedicated medical assistants.
