NURSE PRACTITIONER - Small Panel- High Acuity

Collaborate with 3-5 established PCPs to co-manage a small panel of high-risk adult patients in a primary care practice, providing enhanced, high-touch, independent, and collaborative care. Deliver exceptional quality primary care through extended visits with significantly lower visit volumes than standard practices. Work within the Johns Hopkins SHARP model to improve community health outcomes. Schedule includes 32 hours of clinical time and 8 hours of admin time, with night call once per month.
