Lead high-acuity medical bedside care for residents admitted post-hospital, conducting comprehensive admission assessments within 24–48 hours to manage complex medical transitions and discharge planning. Collaborate with physicians, therapy, and social work to streamline care episodes while ordering diagnostic tests, managing medication reconciliation, and performing minor bedside procedures. Ensure all documentation meets CMS Five-Star Quality Rating requirements and partner with the Medical Director for regulatory readiness. Act as the primary clinical liaison for families to align interventions with goals of care. Operate Monday–Friday with shared telephonic on-call rotation to support 24/7 stability.
