Nurse Practitioner

Part-time Nurse Practitioner or Physician Assistant to conduct in-home health evaluations for Signify Health, including a 45-minute visit with health questions, medication and medical history review, physical evaluation, and applicable diagnostic tests (e.g., spirometry, DEE, bone density screening). Duties include visiting patients in their homes nationwide, connecting with health plan members to bridge gaps in care, utilizing an iPad and clinical workflow, traveling locally as needed, and conducting virtual visits as needed; role emphasizes patient education, care coordination, and delivering personalized health evaluations without prescribing medications or altering treatment plans. Primary locations include Augusta, ME and Boston, MA, with flexible travel to licensed states; part-time benefits and malpractice insurance are provided.
