Job Summary
Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and diagnostic tests like spirometry or diabetic eye exams. Perform virtual visits as needed and deliver Diagnostic & Preventive Services products such as bone density screening. Utilize an iPad and intuitive clinical workflow for each evaluation while connecting one-on-one with patients in their private residences. Work locally within licensed states, covering Grand Junction MSA, with flexibility to travel as required. This full-time role supports Signify Health's mission to bridge care gaps by providing personalized health evaluations directly to members' homes.
