Job Summary
Part-time Nurse Practitioner or Physician Assistant needed for in-home health evaluations with Signify Health (CVS Health). Conduct ~45-minute home visits including medication and medical history review, physical evaluation, and, if required by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams). Use an iPad and a clinical workflow; no prescribing, ordering labs, or altering current treatment. Travel locally within licensed states and conduct virtual visits as needed. Provide diagnostic and preventive services as indicated (spirometry, DEE, bone density screening, etc.). Active, unrestricted licenses, board certification, and ability to cover Augusta, ME (primary) and to travel within licensed states. Part-time benefits include malpractice coverage, supplies, and a robust benefits package. Anticipated weekly hours: 40.
