Job Summary
Nurse Practitioner or Physician Assistant needed for Signify Health’s in-home health evaluations. Clinicians conduct 45-minute home visits to review medication and medical history, perform a physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams). Use an iPad and established workflow; prescribing, ordering labs, or changing current regimens is not required. Flexible to travel locally and within licensed states; virtual visits may be conducted as needed. Provides Diagnostic & Preventive Services and connects members with appropriate care, delivering personalized health evaluations at patients’ homes.
Required Qualifications
- Active, unrestricted license in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
