Job Summary
In-home Health Evaluations for Signify Health (CVS Health) — Part-time Clinician (Nurse Practitioner or Physician Assistant). The role conducts 45-minute in-home assessments including medication and medical history review, physical evaluation, and potential diagnostic tests. Clinicians use a tablet-based workflow (iPad) and are not required to prescribe meds, order labs, or alter treatment plans. Responsibilities include building trusted relationships, visiting members in their homes, coordinating care, and occasionally conducting virtual visits. Travel locally within licensed states is required, with primary coverage in Bellingham and surrounding area. Part-time position with eligible benefits and potential additional compensation through diagnostic/preventive services orders.
