Job Summary
Provide in-home health evaluations for health plan members, delivering a patient-centered assessment in personal residences. Responsibilities include conducting a medication and medical history review, performing a physical evaluation, and, when ordered by the health plan, administering additional diagnostic tests. Use an iPad and a streamlined clinical workflow during each evaluation; participation may include virtual visits. Clinicians join a national network dedicated to improving health outcomes, flexibly travel locally, and collaborate with Signify Health to connect members with the right care. Roles are part-time, with the primary workplace in members' homes, which may expose clinicians to varying environments and household conditions. The position supports NBME-standard care delivery and does not require prescribing, ordering lab tests, or altering patients’ current treatment regimens. Required qualifications include an active, unrestricted license, board certification, and eligibility for practice in the anticipated locations (Cambridge, MA and Strafford County, NH); multi-state licensure is preferred.
