Job Summary
Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical evaluations, and diagnostic tests such as spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to connect one-on-one with patients in their private residences to answer health questions and bridge care gaps. Perform virtual visits as needed and provide all Signify Health services including preventive screenings. Travel locally and within licensed states as required. Work part-time with an anticipated weekly schedule of 29.5 hours.
