Job Summary
Part-time clinician role providing in-home health evaluations for Signify Health (CVS Health) with visits ~45 minutes long. Responsibilities include one-on-one patient engagement, medication and medical history review, physical evaluation, potential diagnostic tests (e.g., spirometry, DEE, bone density screening) as ordered by health plan, and use of an iPad/clinical workflow. Role supports in-home, in-person, and virtual visits; requires active NP or PA licensure, board certification, and ability to travel locally within licensed states. Primary location Augusta, ME; travel and remote-visit capabilities noted; pay range $37.66-$81.11 hourly depending on factors.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover Augusta, ME primary location
- Anticipated Weekly Hours 40
- Part-time employees qualify for benefits
- Experience as NP or PA in home health or in-home evaluations (preferred)
- Strong communication and caregiving skills
- Ability to conduct virtual visits as needed
- Ability to travel locally within licensed states as needed
