Job Summary
Conduct comprehensive patient assessments, diagnose and manage acute and chronic conditions, and develop individualized treatment plans for residents in a Skilled Nursing Facility. Educate patients, families, and staff on disease management and preventive care while maintaining accurate documentation in compliance with regulatory guidelines. Collaborate with physicians and interdisciplinary teams to ensure continuity of care and quality patient outcomes in Fairfax, VA. Full-time and part-time positions available with a highly flexible schedule.
Required Qualifications
- Active Nurse Practitioner (NP) license in the state of Virginia
Desired Qualifications
- Experience in a Skilled Nursing Facility (SNF), long-term care, or geriatric setting preferred
