Job Summary
Provide primary care to PACE participants through physical assessments, semi-annual reassessments, and development of individualized plans of care. Evaluate complaints, treat physical issues, and educate participants and caregivers. Function as a member of the Interdisciplinary Team to coordinate care with physicians and medical directors. Conduct comprehensive histories and physicals for new enrollees, integrate treatment plans, and participate in discharge planning. Perform oversight and medical care for residents of VOANS Skilled Nursing Facilities. Monday through Friday, 8:00a – 4:30p, with rotating on-call after hours and occasional weekend travel to participants' homes.
Required Qualifications
- Licensed as Registered Nurse (R.N.) in the state of employment
- Graduate of a school of professional nursing
- Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing
- A minimum of one year's experience in working with the frail and elderly population
- Either training or related experience in the job assigned
- Monday - Friday, 8:00a - 4:30p, rotating on-call after hours
- Fully onsite
- Occasional weekends
- Intermittent local travel to participants' homes
Desired Qualifications
- Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority
- Two (2) years' experience as a nurse practitioner in a geriatric setting
