Job Summary
Deliver personalized compassionate medical care primarily to patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD). Responsibilities include conducting assessments, counseling patients, maintaining patient records, coordinating care with other providers, and prescribing medications. The role requires effective communication with patients and family members, adherence to quality care standards, and participation in integrated care team meetings. Qualifications include active RN and Nurse Practitioner licenses, BLS certification, DEA certificate, and 2+ years of direct patient care experience, preferably in managed care settings.
Required Qualifications
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- Basic Life Support (BLS) certification
- 2 or more years of direct patient care required
- Ability to work without direct supervision and practice autonomously
- Access to transportation, a valid driver's license, and car insurance
- Proficiency with medical instruments and equipment
- Knowledge of computer-based data management programs
- Ability to communicate effectively in verbal and written form
- Sound understanding of all federal and state regulations including HIPAA and OSHA
Desired Qualifications
- Bilingual (English/Spanish) highly preferred
- Managed Care/IPA/Health Plan experience
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
