Job Summary
Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant at a skilled nursing facility or post-acute rehab center. Collaborate with PCP teams and interdisciplinary care teams including PT, OT, and SLP to manage conditions such as CVA, progressive neurological disorders, and postoperative orthopedic surgeries. Track therapy patient progress and assess barriers to therapy including joint pain, spasticity, and neurogenic bowel or bladder. Represent CRC through strong clinical leadership and collaboration with facility staff while attending multidisciplinary meetings to bridge therapy and internal medicine. Work alongside a remote personal medical assistant and virtual scribe to optimize workflow and documentation.
Required Qualifications
- Nurse Practitioner/APRN/Physician Assistant License
- Michigan License
- Ability to work in the U.S.
- NP/APRN/PA
- Reliably commute or planning to relocate before starting work
