Job Summary
Nurse Practitioner – RCC IP (Temporary Full Time) at Reactivation Care Centre, Etobicoke, ON. The NP will function in an expanded, autonomous role within an interprofessional team to conduct comprehensive health assessments, diagnose and provide treatments (including pharmacological and non-pharmacological therapies), develop and document outpatient care plans, provide patient and family education, and follow-ups, in collaboration with physicians and other team members. Responsibilities include coordinating care, leading quality initiatives, evaluating patient outcomes, and contributing to development of standards of care within the specialty population. Requires RN Extended Class in Adult care, current CNO standing, and BCLS, with demonstrated excellence in medically complex and complex continuing care settings and strong leadership and communication skills.
Required Qualifications
- Registered with the College of Nurses of Ontario in the Registered Nurse Extended Class with clinical focus in Adult care
- In good standing with the College of Nurses of Ontario
- Demonstrated excellence with medically complex care practice and Complex Continuing Care for a minimum of 2 years
- Demonstrated experience with completing comprehensive assessments on complex patients
- Experience in evaluating quality of care and patient outcomes within the specialty population, in partnership with the physicians and other members of the interprofessional team
- Demonstrated effective leadership skills
- Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agencies
- Current Basic Cardiac Life Support (BCLS) certification
