Nurse Practitioner - PT (Nights & Weekends)

Job Summary
Part-time Nurse Practitioner conducts in-home wellness assessments for pediatric and adult members (in-home annual visits up to one hour). Responsibilities include evaluating medical history and health status, assessing psychosocial needs, coordinating care plans with primary care and providers, ensuring HIPAA compliance, and providing education to members and caregivers. Travel up to 90% within designated regions; in-home Well Child visits (6 months–21 years) and adult wellness exams; strong emphasis on preventive care, social determinants of health, and cost-quality outcomes. Availability Monday–Friday 3:00–8:00 pm and weekends 8:00 am–5:00 pm; minimum 10 hours/week.
Required Qualifications
- Master's Degree in Nursing
- Current and unrestricted licensed Family Nurse Practitioner
- Current CPR certification
- Current BLS certification
- Annual TB testing
- One (1) year of NP experience preferred
- Experience in care of the elderly preferred
- Experience in home health, physician’s office, or public health is preferred
