Job Summary
Part-time Nurse Practitioner to conduct in-home wellness assessments for pediatric and adult members, including in-home Well Child visits and adult wellness exams. responsibilities include evaluating medical history, health status, and social determinants of health; coordinating care plans with primary care and community providers; educating members on health conditions; performing IHRA; traveling up to 90% within designated regions; flexible weekday and weekend evening hours; ensuring HIPAA-compliant documentation and ongoing care coordination.
Required Qualifications
- Master's Degree in Nursing
- Current and unrestricted licensed Family Nurse Practitioner
- Current CPR certification
- Current BLS certification
- Annual TB testing
