Job Summary
Conduct patient rounds and manage psychiatric care across Skilled Nursing Facilities, Long-Term Care, Assisted Living, and Independent Living settings. Partner with attending physicians, specialists, and facility staff to deliver personalized psychiatric services, ensuring continuity of care across the post-acute and long-term care continuum. Assess and manage the mental health needs of residents, perform physical exams, order and interpret diagnostic tests, and develop therapeutic care plans. Prescribe and administer medications and treatments, including necessary injections, while educating patients and families on preventive mental health practices. Address acute psychiatric and medical needs promptly within a collaborative, multidisciplinary team environment.
Required Qualifications
- Graduation from an accredited Nurse Practitioner program
- ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active DEA license
- BLS and ACLS certification
- Current Nurse Practitioner license in the state of practice
- Valid driver's license
- Current auto insurance
- Acceptable driving record
