Job Summary
Conduct patient rounds and manage psychiatric care across Skilled Nursing Facilities, Long-Term Care, Assisted Living, and Independent Living communities. Partner with attending physicians, specialists, and facility staff to assess mental health needs, perform physical exams, and develop therapeutic care plans. Order and interpret diagnostic tests, prescribe medications, and provide injections as necessary while educating patients and families on preventive practices. Collaborate with multidisciplinary teams to address acute psychiatric and medical needs promptly, ensuring continuity of care across the post-acute and long-term care continuum.
Required Qualifications
- Graduation from an accredited Nurse Practitioner program
- ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active DEA license
- BLS and ACLS certification
- Current Nurse Practitioner license in the state of practice
- Valid driver's license
- Current auto insurance
- Acceptable driving record
