Nurse Practitioner - Psychiatric

Conduct psychiatric patient rounds and manage care across Skilled Nursing, Long-Term Care, Assisted Living, and Independent Living communities. Collaborate with attending physicians, specialists, and facility staff to coordinate comprehensive mental health services, perform physical exams, and interpret diagnostic tests. Prescribe medications, administer treatments including injections, and develop therapeutic care plans for residents and families. Educate patients on preventive practices and manage acute psychiatric needs through multidisciplinary team support. This role requires travel to assigned facilities and offers autonomy within a data-driven care model focused on improving outcomes for older adults.
