Job Summary
Nurse Practitioner within the Institutional Special Needs Plan (ISNP) to provide coordinated, preventive and comprehensive care to PPHP members at designated skilled nursing facilities. Partners with the member’s PCP to deliver high-quality care, serves as the medical liaison to interdisciplinary staff, orders and interprets tests, prescribes medications, and develops short- and long-term care plans. Responsibilities include autonomous comprehensive health assessments, cross-functional care plan development focusing on prevention and timely intervention, adherence to PPHP documentation standards, maintaining scope-of-practice compliance, and effective communication with physicians, families, and care teams. Requires 3+ years of clinical care experience and current NP licensure (Adult/Gerontological or Family) with credentialing, DEA/CDS eligibility, BLS, and an unrestricted RN license; valid driver’s license and reliable transportation. Location is Kokomo, IN with on-site facility work.
Required Qualifications
- Minimum of 3 years clinical care experience.
- Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice.
- Meets Provider Partner Health Plan credentialing requirements.
- Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice).
- Basic Cardiac Life Support (BCLS) certification.
- Current unrestricted RN license for the state in which the Nurse Practitioner is practicing.
- Valid driver’s license and car insurance with reliable transportation.
