Job Summary
Conduct adult and geriatric health assessments at members' homes or skilled nursing facilities, reviewing medical history, medication adherence, and social environment while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, and caregivers to provide health education, manage referrals, and ensure clear documentation per Matrix protocols. Perform these visits Sunday through Saturday between 8 am and 7 pm, with flexible scheduling based on your availability. Complete a monthly average of 20 to 100 visits to earn between $1,800 and $11,500, including mileage reimbursement and 401(k) benefits.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice
- Prescription authority or the ability to obtain prescriptive authority
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills
- Familiarity with employee health/medical record software
- Excellent verbal and written communication skills
