Job Summary
Conduct adult and geriatric health assessments at members' residences or Skilled Nursing Facilities, reviewing medical history, medication adherence, and social environments. Administer point of care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers on patient education and referrals. Provide clear documentation and follow-up in accordance with Matrix guidelines during visits lasting up to one hour. Schedule availability between 8 am and 7 pm, Sunday through Saturday, with flexible day, evening, or weekend shifts. Requires current NP licensure with prescription authority, a Master's degree, and BLS/ACLS certification.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
