Job Summary
Provide in-home Nurse Practitioner assessments and health management for members and government programs. Conduct focused and comprehensive physical exams, obtain detailed medical histories, interpret diagnostic information, perform chronic disease assessments, medication reviews, and preventive screenings. Document findings in electronic medical records, educate patients and caregivers on self-management and preventive services, and coordinate with health plans and primary care providers. Collaborate with a multidisciplinary team to close care gaps, improve quality metrics, and enhance member satisfaction while delivering autonomous, compassionate, one-on-one care in patients' homes or community settings.
Required Qualifications
- Current Nurse Practitioner license
- Board-certified Nurse Practitioner or eligibility
- Ability to travel to patients' residences for in-home evaluations
- Experience with in-home or community-based care preferred
