Nurse Practitioner - PRN

Nurse Practitioner (NP) - PRN position with potential to transition to full-time opportunities in Matrix Medical Network's in-home health assessment program. Conduct adult/geriatric assessments during visits at members’ residences or skilled nursing facilities, review medical history, medication adherence, social environment and other health risks, administer point-of-care and lab testing, provide health education, and coordinate with physicians, case managers, social workers, families, and other care teammates. Projects include documentation per Matrix guidelines, collaboration with PCP on follow-up and education, and scheduling across Sundays–Saturdays (8 am–7 pm) based on availability. Competitive pay is tied to the number of assessments completed (monthly averages reported as $1,800–$11,500 for 20–100 visits per month). Requires Master’s degree in nursing or commensurate experience, NP licensure, board certification in a relevant specialty (Adult, Adult Gerontology, Family, etc.), BLS/ACLS/CPR certification, and a valid driver’s license with appropriate auto insurance.
