Job Summary
Conduct adult and geriatric health assessments at member residences or Skilled Nursing Facilities, reviewing medical history, medication adherence, and social risks to identify care gaps. Administer and order point-of-care and lab-based testing as needed while collaborating with physicians, case managers, social workers, and family caregivers. Provide patient education, manage physician referrals, and ensure clear documentation according to Matrix guidelines during visits lasting up to one hour. Schedule availability between 8 am and 7 pm, Sunday through Saturday, with pay based on assessment completion rates ranging from $90 to $115 per visit.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license
- Proof of adequate automobile insurance coverage for the state of residence
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
