Job Summary
Nurse Practitioner (PRN) conducting comprehensive in-home health assessments (CHA) for adults/geriatric patients, visiting members at their residence or Skilled Nursing Facilities, performing medical history reviews, health education, referrals (physician, case management), administering point-of-care and lab testing, and collaborating with physicians, social workers, and caregivers. Roles offer flexible scheduling (in-home visits Mon–Sun 8am–7pm), potential transition to full-time locations, mileage reimbursement, and opportunities for credentialing support and continuing education. Effective NP licensure, board certification, and driving authorization are required to provide home-based assessments and coordinate follow-up care.
Required Qualifications
- Master's Degree in Nursing
- NP licensure in state of practice with prescriptive authority or ability to obtain
- AANP AACN or ANCC board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year NP experience preferred, new grads encouraged
- Valid state driver's license and car insurance
- Strong computer skills with medical records software
