Job Summary
NP with state licensure, reviews and conducts adult/geriatric comprehensive health assessments at member homes or Skilled Nursing Facilities, including medical history, diagnoses, treatment, health education, referrals, and documentation; administers point-of-care and lab testing; collaborates with PCP, case managers, family members, and other clinicians; provides care coordination and follow-up; flexible scheduling (Visits Sunday–Saturday 8am–7pm) and potential transition to full-time; requires board certification in adult/gerontology or family practice, BLS/ACLS, and strong computer skills; insured mileage reimbursement and ongoing education support.
Required Qualifications
- Master’s Degree or commensurate NP experience with licensure
- Current NP licensure in state of practice with prescriptive authority or ability to obtain
- AANP, AACN, or ANCC board certification in applicable NP specialty
- Current BLS, ACLS or CPR certification
- 1 year NP experience preferred; new grads encouraged to apply
- Valid state driver’s license and automobile insurance for state of residence
- Strong computer skills and familiarity with medical records software
- Excellent verbal and written communication skills
