Job Summary
Nurse Practitioner (PRN) providing in-home clinical assessments and health management for health plan members and government-sponsored participants. Travel to patients’ residences or community settings to perform focused and comprehensive physical exams, chronic disease assessments, medication reviews, preventive screenings, detailed medical histories, and patient/caregiver education on self-management, adherence, lifestyle changes, and preventive services. Document findings in electronic medical records, communicate care needs to health plans and primary care providers, and help link patients with appropriate resources to improve health outcomes and reduce avoidable hospitalizations. Collaborate with a multidisciplinary team to identify risks, close care gaps, follow standardized protocols, support quality metrics, and contribute to value-based care models. Emphasizes autonomy, one-on-one patient interaction in the patient’s environment, and adherence to clinical guidelines and regulatory and payer requirements. Flexible scheduling in a mission-driven organization focused on improving patient outcomes and reducing healthcare costs through proactive, in-home care.
