Job Summary
Conduct adult and geriatric health assessments at member residences or Skilled Nursing Facilities, reviewing medical history, medication adherence, and social environments to identify care gaps. Administer and order point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers on patient education and follow-up. Provide clear documentation according to Matrix guidelines and protocols during visits lasting up to one hour. Schedule availability allows work days, evenings, and weekends from 8 am to 7 pm, with compensation based on assessment completion rates ranging from $90 to $115 per visit. Requires Master's degree, current NP licensure with prescription authority, and BLS/ACLS certification.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice
- Prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills
- Familiarity with employee health/medical record software
- Excellent verbal and written communication skills
