Job Summary
Provide comprehensive health assessments for members in their homes or Skilled Nursing Facilities as part of Matrix Medical Network's in-home health assessment program. Responsibilities include conducting adult/geriatric assessments (medical history, diagnoses, treatment, health education, referrals), ordering point-of-care and lab tests, coordinating with physicians, case managers, social workers, and caregivers, and ensuring clear documentation per Matrix guidelines. Services occur at the member’s residence with flexible scheduling (weekdays and weekends). Opportunities include potential transition from PRN to full-time roles, competitive pay based on assessments completed ($1,800–$11,500 per month on average), mileage reimbursement, and ongoing education/credentialing support. Must hold NP licensure with prescriptive authority (or ability to obtain) and current certifications (BLS/ACLS or CPR). Required Qualifications
- Master’s Degree or commensurate experience and satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience preferred, new grads encouraged to apply!
- Valid state driver’s license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
