Job Summary
Conduct adult and geriatric health assessments at members' residences or skilled nursing facilities, reviewing medical history, medication adherence, and social environments. Administer and order point-of-care and lab-based testing as needed while documenting findings according to Matrix guidelines. Collaborate with physicians, case managers, and caregivers on patient education, referrals, and follow-up to close care gaps. Perform visits lasting up to one hour, with flexible scheduling available Sunday through Saturday from 8 am to 7 pm. Compensation ranges from $1,800 to $11,500 monthly based on assessment volume and location. Master's degree, current NP licensure, and relevant board certification are required.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
