Job Summary
Consult with patients and families regarding disease processes, self-care techniques, and end-of-life care. Assist in developing patient and family plans of care to ensure quality service. Collaborate with attending and consulting physicians regarding palliative needs and inform them of changes in patient outcomes. Submit all documentation timely and ensure compliance with local, state, federal laws, Medicare regulations, and personnel policies. Maintain a current driver's license and provide valid auto liability insurance. Work as a PRN hourly nurse practitioner in a community-based palliative care program, with travel required.
Required Qualifications
- Graduate of a Baccalaureate School of Nursing
- Current State license as a Registered Nurse and as an Advanced Practice Nurse (APN)
- Current Driver's License

- Two years of experience as an APN in a clinical care setting
- Comprehensive knowledge of pharmacology
- Comprehensive knowledge of palliative care
- Comprehensive knowledge of end-of-life
- Ensure overall compliance with local, state and federal laws
- Ensure overall compliance with Medicare regulations
- Ensure overall compliance with established personnel policies and procedures
