Job Summary
Conduct adult and geriatric health assessments in member homes or skilled nursing facilities, reviewing medical history, medication adherence, and social risks to identify care gaps. Administer point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers to provide education and referrals. Document findings per Matrix guidelines and coordinate follow-up with primary care providers. Schedule visits between 8 am and 7 pm across the week based on availability, earning $1,800 to $11,500 monthly based on assessment volume. Requires Master's degree, current NP licensure with prescription authority, and valid driver's license.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice
- Prescription authority or the ability to obtain prescriptive authority
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage for the state of residence (or ability to obtain)
- Strong computer skills
- Familiarity with employee health/medical record software
- Excellent verbal and written communication skills
