Job Summary
Conduct adult and geriatric health assessments in member homes or skilled nursing facilities, reviewing medical history, medications, and social risks while administering point-of-care and lab testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide patient education, and ensure clear documentation per Matrix protocols. Travel up to 80-90% of the time across the assigned state, including overnight trips and weekends, to complete assessments that identify chronic conditions and close care gaps.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP experience
- Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
